opening ports to host in WC3

i have a Netgear WNR854T and i am trying to open to proper ports to host games in WC3, i opened some of the ports needed, but it asks for a server IP address aswell... can someone give me the exact port numbers along with the server IP address i need?
 
The server IP address is the address for your computer. As for the ports, it should be in a read-me on the CD.
