Folders Not In Alphabetical Order

Just recently (...not sure how I managed to do this), but every time I go to save a file, instead of all of my files and folders being in alphabetical order, they're splattered all over the place. I then have to right click and "Arrange Icons By Name" in order to put them back in alphabetical order.

What exactly did I do wrong to make this happen? And of course, how can I fix the problem?

p.s. This is only when I go to save a file, not when I actually click on a folder.
 
That's what I do. But I don't want to have to do that every time I go to save a file, you know? I use to navigate to a folder and they (files and folders) would be in alphabetical order. Now I go and I have to, as you say, change the sort order there every single time. It's annoying.
