cpu is never idle

I've been having this problem for about a week now. The % of cpu usage jumps all over from 3% to 80% constantly. When programs are actually running it will be especially high but it is high even when nothing is running. I've run a few spyware and virus scans, cleaned up the hard drive, removed old junk programs, minimized what was on the startup. I'm out of ideas and it's getting to be a real headache waiting 2min for a window to open. Sometimes it runs like normal but more often than not it will be super laggy. Pleas help me fix this!!!
 
I did defrag but i was hoping it wouldn't come down to a re install. It's such a time killer to set everything up again.
 
Knowing your system specs and what OS you have would be good to know for starters. Vista on a single-core? I wouldn't be surprised. XP on a brand new quad? We have a problem.
